Order Entered: 1. Within 20 days of the date of entry of this Order, Big Sandy District shall publish, in a newspaper of general circulation in Boyd County, the notice of the vacancy on Big Sandy District's Board attached as Appendix hereto. 2. Within five days of the publication, Big Sandy District shall supply proof of publication to the Commission. 3. Within 20 days of the date of publication, any person who wishes to apply as a candidate to fill the vacancies on Big Sandy District's Board of Commissioners shall submit to the Commission, in writing, their name, address, telephone number, qualifications and shall reference Case No. 2019-00187. 4. The term of each commissioner appointed shall be four years pursuant to KRS 74.020, and the terms of current commissioners shall be extended to four years and staggered upon the filling of vacancies to be determined upon final order. 5. The Executive Director shall serve a copy of this Order upon the Boyd County Judge/Executive; the members of the Boyd County Fiscal Court, the Carter County Fiscal Court, the Johnson County Fiscal Court, the Lawrence County Fiscal Court; the Boyd County Attorney; and the Big Sandy District's Board.

Order Entered: 1. This proceeding is initiated to determine whether a vacancy existed on the Big Sandy Water District's Board of Commissioners for more than 90 days. 2. Big Sandy Water District is made a party to this proceeding. 3. Any party filing documents with the Commission shall serve a copy of those documents upon all other parties. 4. a. The information requested in Appendix B to this Order is due within 14 days of entry of this Order. Responses to requests for information shall be appropriately bound, tabbed, and indexed and shall include the name of the witness responsible for responding to the questions related to the information provided, with copies to all parties of record, and the original and five copies to the Commission. b. Each response shall be answered under oath or, for representatives of a public or private corporation or a partnership or association or a governmental agency, be accompanied by a signed certification of the preparer or the person supervising the preparation of the response on behalf of the entity that the response is true and accurate to the best of that person's knowledge, information, and belief formed after a reasonable inquiry. c. A party shall make timely amendment to any prior response if it obtains information which indicates that the response was incorrect when made or, though correct when made, is now incorrect in any material respect.
